- During inspiration, the negative intrathoracic pressure increases
venous return to the right heart. The right atrium accepts the increased
venous return which in turn decreases the inspiratory jugular venous
pressure (JVP).
In contrast, constrictive pericarditis reduces the distensibility
or compliance of the right atrium and elevates the intracavitary pressure.
This prevents the chamber from accepting the entire increase in blood
flow during inspiration. This resistance to filling of the chamber causes
the JVP to rise during inspiration.This
is known as Kussmaul's sign.
